Operator's Report of Motor Vehicle Accident

• Use this form (click on link to obtain copy) to report a motor vehicle accident on or after November 1, 2001.
• Obtain four copies:
  • Send the first copy to the police department in the jurisdiction where the accident took place.
  • Send the second copy to the Registry of Motor Vehicles
  • Retain the third copy for your records.
  • Send to Insurance Company
