BLOOD SUGAR
Generally the second Wednesday of the month from 9:00 -10:30AM.
Fasting is recommended.

FOOT CARE
Conducted by PEDI-CARE, LLC the third Friday of the month Registered nurses provide pedicures and foot care including toe nail trimming & filing, smoothing of corns & calluses; stimulating massage. An appointment is needed. Call 565-4150. Call 567-1023 for an appointment for home visits.

FLU CLINIC
Conducted in cooperation with the Board of Health annually in October. Medicare accepted or $1.00 donation. Home visits available for the homebound. Call 565-4150 for details.

PNEUMONIA CLINIC
In cooperation with the Board of Health, the Senior Center offers a clinic in October/ November annually. Appointments necessary through the Board of Health - 565-4140.

TOTAL LIPID PROFILE
A total lipid profile with cardiac risk assessment is held semi-annually. Generally held in April & September.

URINALYSIS
Urinalysis is done by appointment. Call 565-4150. Cost $1.00 for supplies used.

ADULT FOSTER CARE: a statewide program (MA) which provides a living arrangement in which an eligible elder is matched with a family. Assisted Living

CONGREGATE HOUSING - a state funded program (MA) designed to provide a supportive living environment--characterized by common living areas such as kitchen, dining room, rec room--for frail, eligible elders. There are also private Congregate Housing situations.

GSSSI is a multi-service state home care agency which works to help low-income, frail elders so that they may remain living independently at home. Covers the towns of Agawam, Brimfield, East Longmeadow, Feeding Hills,, Hampden, Holland, Longmeadow, Monson, Palmer, Springfield, Wales, West Springfield, Wilbraham.

Eligibility Criteria:
Participant must be 60 years of age or older.
The applicant must meet frailty standards that have been defined by the Executive Office of Elder Affairs (EOEA). Assistance needed with bathing, dressing, eating, transferring, toileting and incontinence maintenance, meal preparation, housework, laundry, shopping, taking medicine, transportation, money management are a sampling or need criteria established by EOEA. HOME OWNERS ASSISTANCE PROGRAMS
HOME Program - Homeowner Options for Massachusetts Elders

HOME is a statewide nonprofit housing counseling agency. It serves older homeowners with limited income and assets. HOME has a network of counselors who make home visits to assess the needs of elders and match them with a wide variety of community resources in order to preserve their homes and independence. HOME also counsels and assists elder with the Reverse Mortgage Program whereby funds flow form lender to consumer. SELOC (The Senior Home Equity Line of Credit) is another program HOME offers. Eligible elders can receive a line of credit. Along with this line of credit comes guidance from a HOME counselor as to whether this program is the best decision based on the elders needs.

HOME REPAIR LIST - This is a paid referral service for seniors to use at home. Call the Senior Center for information at 565-4150. Volunteers are available for odd jobs.

HOUSING AUTHORITY GUIDELINES
In Massachusetts the Housing Authority administers state housing projects which provide affordable housing for those who meet eligibility criteria. Special programs include: 705 family program, Mass rental voucher program, handicapped and senior housing. Call desired housing authority or complex for application, specific requirements, and availability. Income/Asset eligibility may vary by town, complex or housing authority depending on source of subsidy. In general, one must be low income and/or elderly (60+) and/or handicapped. Approximate income guidelines (subject to change) for elderly housing are as follows: State & Federal Housing - $23,900/single: $27,350/couple with no asset limit.

REAL ESTATE TAX EXEMPTIONS - Town of Longmeadow
Clause 41C - Elderly Persons - 70 years of age or older
A tax exemption is a discharge from the obligation to pay all or a portion of tax. Exemptions are conferred by the Legislature on particular categories of persons or property. The exemption amount for Clause 41C is $500. Applications must be filed annually on or before December 15 with the local assessors in the city or town where the property is located. If the tax bill is mailed after September 15, however, applications may be filed within three months form the date the bill was first mailed. Filing an application does not entitle one to a delay in tax payment. An applicant for an exemption must provide to the assessors whatever information is reasonably required to establish eligibility. This information may include, but not be limited to: birth certificates: Evidence of domicile and occupancy and income tax returns. For eligibility, an individual must satisfy requirements relating to age, ownership and domicile, annual income, whole estate or assets. Not more than one exemption may be granted under Clause 41C on the same parcel of real estate. An individual must be 70 years or older or joint owner with a spouse 70 years or older as of July 1 or the tax year. Under Clause 41C, an individual must own and occupy the subject property on July of the tax year and the individual must have been continuously domiciled in Massachusetts for the 10 years preceding the application and have owned and occupied the property or other property in Massachusetts for 5 years. Annual Income: Gross Receipts minus social security allowance must be less than: $13,000 if single, $15,000 if married. Whole estate less than value of the home except for the value of any possession which exceeds three dwelling units and produces income cannot exceed: $28,000 if single and $30 if married. CLAUSE 37 - BLIND PERSONS
Exemption Amount - $437.50
Applications must be filed annually on or before December 15 with the local assessors in Longmeadow. If the tax bill is mailed after September 15, however, applications may be file within three months from the date the bill was first mailed. Filing an application does not authorize an applicant to delay making a tax payment. Not more than one exemption may be granted under Clause 37 on the same parcel of real estate. For eligibility, an individual must satisfy requirements relating to proof of blindness and ownership and domicile. For further information contact 565-4115.

QUALIFYING VETERANS
Clause 22, 22A, 22B, 22C, 22D, 22E, Paraplegics
Applications must be filed annually with the local assessors I the city or town where the property is located on or before December 15th, or three months after the actual tax bills are mailed, whichever is later. Filing an application does to entitle the applicant to a delay in tax payment. For further information on eligibility and exemption amounts contact the Longmeadow Assessors office at 565-4115.

TERRIFIC TEENS - This is a referral service for seniors which teens may earn some extra money doing odd jobs such as raking, mowing, shoveling, moving an item, etc. Call the Senior Center for information at 565-4150.
