will get involved in patient/doctor dispute mediation/resolution.
Call to register a complaint against a physician - if the physician
is a member of the Hampden Medical Society. If the physician
in question is not, call the Board of Registration in Medicine

Support groups
offer a way to address a loss or significant change in health
or circumstance. Often family members, or an elder, find joining
a support group helpful. The following is a partial listing for
the Greater Springfield area. Call for information as meeting
dates are subject to change.

TRANSPORTATION CONCERNS
Pioneer Valley
Transit Authority (PVTA)
The Longmeadow Mini-Van provides transportation to residents
over the age of 60 and those who are physically challenged. The
service operates, Monday through Friday, 8:00 am - 4:30 pm. Call
the Adult Center at 565-4150 for details.
Longmeadow and surrounding communities are accessible through
the mini-bus as are medical appointments, errands, hairdressers,
shopping and trips to the Senior Center.

HANDICAPPED PLATE/PLACARD
call the Medical Affairs Office at 617-351-9222. For plates/placards
an application must be filled our by applicant and a physician.
Applications can be picked up at , or mailed to you from, the
local Department of Motor Vehicle office. The Medical Affairs
Office will process applications for handicapped plates/placards
in about 30 days. Call or write them at 1135 Tremont St., Boston,
MA 02120.

PIONEER VALLEY
TRANSIT AUTHORITY

PVTA provides door to door transportation to the elderly and
disabled in Hampden and Hampshire Counties through the local
Paratransit Van service.
ADA - Americans
with Disabilities Act of 1990 provides transportation through
PVTA. If you have a disability which prevents you from accessing
the PVTA bus, you may be eligible for ADA Paratransit Van service.
To access this service, an application must be submitted to the
PVTA. Contact the PVTA or Council on Aging for an application
(565-4150).
Anyone who wishes
to access PVTA transit bus transportation must obtain a PVTA
photo ID pass card at the PVTA Information Center, 1500 Main
Street, Bay State West, Springfield.
