The Longmeadow Board of Health has scheduled fall flu clinics open to all Longmeadow residents over 18 years of age.
Appointments are required and can be made here or by calling the Board of Health at (413) 565-4140. Walk-in appointments will NOT be accepted.
Immunization forms are available on this website and residents are strongly encouraged to fill out the forms in advance and bring them to their scheduled appointment. A copy of your health insurance card must also be brought to the appointment. Forms will be available at the clinic and can be completed onsite if needed.
If you do not have insurance or are required to pay for your flu shot, please bring $6.00 to cover the cost of the vaccine. Payment can be made via cash or check made payable to the Town of Longmeadow.
The clinics will be held at the Greenwood Center, 231 Maple Road, please enter through the main entrance (the entrance to the Child-Care Center) and follow the signs. Masks will be required at all times.

Medicare B, Mass Health, Medicare Advantage Plans, and private health insurance plans cover the cost for flu shots, an insurance card must be brought to your appointment for verification. For individuals with no health insurance or insurance that does not cover flu shots (e.g. Aetna, Humana, and Anthem, CT) please bring $6.00 in the form of cash or check (made payable to the Town of Longmeadow) to cover the cost of the shot.
